Overview

Do you have a passion for helping individuals address medical, behavioral, and/or social needs? Do you have an interest in educating our members on preventive health guidelines and programs?

If so, we would love for you to consider applying for the Member Care Coordinator position. This individual would be responsible for working directly with members on medical, behavioral, or social needs, and updating their care plans accordingly.

Responsibilities

  • Conducts initial and annual assessments of members’ medical, behavioral, and social needs.
  • Reviews, monitors and updates care plans.
  • Educates members on preventive health guidelines and programs.
  • Coordinates and organizes information from members’ assessment to develop a care plan that addresses their needs and assures implemented interventions are discussed with members and their providers/supports.
  • Documents member information and contacts made regarding member’s care and services provided.
  • Provides information to members and providers regarding benefits, service providers and access protocols and educates members on appropriate use of medical services.

Qualifications

  • Bachelor’s degree or Associate’s degree in social service, rehabilitation, psychology, or related field.
  • 3+ years of experience in healthcare or social work.
  • Experience working with Electronic Medical Records is preferred.
  • Healthcare or social work experience is preferred.
  • Experience with Motivational Interviewing and Trauma informed care is preferred.
  • Knowledge of Medicare and Medicaid benefits.
  • Proficiency in speaking Spanish, Hmong, or another foreign language is strongly preferred.
  • Valid Driver’s license, verifiable good driving record, and reliable transportation, with the ability to travel, as necessary, to attend off-site meetings on a regular basis.
