Overview

Come Find Your Spark at Quartz! The Population Health Outreach Specialist will play a critical role in empowering communities, connecting individuals to resources, and improving health outcomes by conducting phone calls and text messaging outreach services to inform members of actions related to recommended tests or screenings or to secure clinical data on behalf of care teams that meet medical, behavioral health, and social needs of Quartz members. This position will build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counselling, social support, and advocacy.

Responsibilities

  • Conducts phone call and text messaging outreach to members in priority populations to improve member experience and clinical, quality outcomes.
  • Organizes information from members’ health history and internal data sources to develop an outreach approach to address member needs and ensure implemented interventions are collaborated with members and their providers.
  • Collaborates with Quartz clinical services, local clinical team, social service providers, and community stakeholders to support the member’s care plan.
  • Provides referrals for community resources and social services.
  • Reviews, monitors, and updates quality program plans, materials, and reports.
  • Documents member information and contacts completed regarding the members’ care and services provided.
  • Actively monitors the members’ ability and willingness to engage in treatment regimes, and problem solve with staff, members, and providers, offer alternatives or solutions to meet their needs.

Qualifications

  • Bachelor's degree with 2 years of experience in healthcare or social work
  • OR associate degree with 5 years of experience in healthcare or social work
  • OR High school equivalency with 8 years of experience in healthcare or social work
  • Able to work independently in Microsoft Office Applications
  • Able to learn documentation systems such as Epic
  • Knowledge of Medicare and Medicaid benefits
  • Effectively communicate thoughts, ideas, and information both orally and in writing
  • Able to demonstrate flexibility, set priorities with daily demands and long-term work assignments and projects
  • Strong interpersonal skills with ability to effectively interact with members and coworkers from a variety of different backgrounds and experiences in a professional and courteous manner
  • Knowledge of local, state, and federal community resources available to members
  • Able to problem solve and provide solutions using appropriate methodologies
  • Exceptional customer service skills including an understanding of the needs of culturally diverse populations
  • Ability to travel to off-site meetings, as required

Hardware and equipment will be provided by the company, but candidates must have access to high-speed, non-satellite Internet to successfully work from home.
